RMA Service Request Form

To request an RMA for your BIRD equipment, please fill out the form below. A Service Center representative will respond to your request within 24 hours. You can Pre-pay your order by providing a PO with your shipment or credit card info below in the comments section. To view "Flat Rate Pricing" (Microsoft Excel required), click the button below. For smaller shipments: Send returns through Federal Express directly to the Bird Service Center.

All invoices should state "US GOODS RETURNED" and include a Value for Customs. If being returned for repair or calibration, please state: "US GOODS BEING RETURNED FOR REPAIR AND/OR CALIBRATION".
